(a) Discontinuer sample defined as women who had not returned for 3 months after scheduled appointment (b) Continuer sample: women: who had attended within the last 3 months Cross-sectional (one-time) study (a) Discontinuers: systematic random sample of all discontinuers at the 13 sample clinics.
